La Malvasía cabeciblanca es una especie globalmente amenazada (IUCN Endangered; BirdLife International, 1998, 2000). Ello ha llevado a BirdLife International a redactar un Plan Internacional de Acción para la recuperación de dicha anátida. Además existen otros planes de acción a escala mundial (Anstey, 1989), europea y del Paleártico Occidental (Green y Hughes, 1996; Hughes et al., 1999), así como nacional (España y Grecia).
La principal amenaza para la especie actualmente es la contaminación genética producida por hibridación con la Malvasía canela (Oxyura jamaicensis). Otra gravísima amenaza para las poblaciones silvestres es el plumbismo, patología derivada de la ingestión de perdigones de plomo como consecuencia de su uso como munición en la caza de aves acuáticas.
También las artes de pesca y la competencia con algunas especies inciden negativamente sobre sus poblaciones.
